I think to prevent an array of these questionnaire topics, we should just have one.

How to do this exactly?

Everyone who posts in this topic creates their own questionnaire, and then they just edit their posts with the Scores members received. Since I'm sure many people would want to do a questionnaire, I think it's in our best interest to do have just one topic, instead of many.
